📄 شرایط و ضوابط ثبت‌نام و همکاری پزشکان در پلتفرم MyMedEx
مقدمه
این سند به‌عنوان «شرایط و ضوابط همکاری پزشکان» در پلتفرم MyMedEx تنظیم شده و کلیه پزشکان متقاضی همکاری موظف به مطالعه، پذیرش و پایبندی کامل به مفاد آن هستند. عضویت در این پلتفرم به منزله پذیرش کامل و بدون قید و شرط مفاد زیر است.
1. مسئولیت‌های حقوقی و حرفه‌ای پزشک
1.1 کلیه مسئولیت‌های قانونی، کیفری و حرفه‌ای ناشی از خدمات درمانی، مشاوره‌ای، تشخیصی یا تجویزی که پزشک از طریق پلتفرم MyMedEx به کاربران ارائه می‌دهد، به‌طور کامل بر عهده پزشک معالج بوده و پلتفرم هیچ‌گونه مسئولیت مستقیم یا غیرمستقیم در این خصوص نخواهد داشت.
1.2 پزشک متعهد است تنها در چارچوب تخصص، صلاحیت علمی و مجوزهای رسمی خود به ارائه خدمات اقدام نماید. در صورت هرگونه تخلف، قصور پزشکی، سهل‌انگاری یا رفتار خلاف اخلاق حرفه‌ای، پزشک شخصاً پاسخ‌گو خواهد بود.
1.3 در صورت شکایت کاربر یا بروز اختلاف، کلیه مستندات و مکاتبات جهت پیگیری قانونی یا ارجاع به هیأت‌های نظارتی و حقوقی مرتبط در کشور محل اقامت پزشک، در اختیار مراجع ذی‌صلاح قرار خواهد گرفت.
2. تعهد به پاسخ‌گویی و خدمات‌رسانی
2.1 پزشک موظف است در بازه‌های زمانی مشخص‌شده در پنل شخصی خود، در جلسات مشاوره آنلاین (متنی، صوتی یا تصویری) شرکت کرده و نسبت به ارائه کامل، دقیق و شفاف خدمات اقدام نماید.
2.2 پزشک متعهد است در صورت بروز هرگونه مشکل، تأخیر یا عدم امکان حضور در جلسه تعیین‌شده، حداقل 6 ساعت پیش از زمان ویزیت، موضوع را از طریق شماره پشتیبانی یا ایمیل رسمی زیر به اطلاع تیم پشتیبانی برساند:

2.3 عدم اطلاع‌رسانی و عدم حضور موجه در زمان ویزیت، تخلف محسوب شده و ممکن است منجر به غیرفعال‌سازی موقت یا دائمی حساب کاربری پزشک در پلتفرم گردد.
3. حفظ حریم خصوصی و امنیت اطلاعات
3.1 پزشک متعهد است کلیه اطلاعات شخصی، پزشکی و پرونده‌های کاربران را محرمانه نگه‌داشته و صرفاً در چهارچوب الزامات اخلاقی و قانونی از آن‌ها استفاده نماید.
3.2 ذخیره، چاپ، ارسال یا اشتراک‌گذاری اطلاعات کاربران بدون رضایت رسمی و مکتوب آنان، ممنوع بوده و در صورت مشاهده، حساب پزشک بلافاصله تعلیق خواهد شد.
4. حقوق مالکیت پلتفرم
4.1 پزشک تأیید می‌نماید که مالکیت معنوی برند MyMedEx و کلیه حقوق نرم‌افزاری، طراحی، سیستم‌ها، مستندات، محتوا و زیرساخت‌های فنی متعلق به شرکت MedExFuture بوده و هیچ‌گونه حقی نسبت به آن نخواهد داشت.
5. فسخ همکاری
5.1 پلتفرم MyMedEx حق دارد در صورت مشاهده هرگونه تخلف، رفتار نامناسب، امتناع مکرر از پاسخ‌گویی، یا نقض مفاد این سند، همکاری با پزشک را در هر زمان و بدون نیاز به ارائه دلیل، به‌صورت یک‌طرفه فسخ نماید.
6. پذیرش نهایی
ثبت‌نام پزشک در پلتفرم و شروع فعالیت به معنای پذیرش کامل این شرایط و ضوابط تلقی می‌گردد. پزشک متعهد می‌شود در تمامی مراحل همکاری، به قوانین و الزامات رسمی کشور محل اقامت خود، اصول اخلاق حرفه‌ای، و مقررات داخلی پلتفرم MyMedEx پایبند باشد.
📄 Terms and Conditions for Physician Registration on the MyMedEx Platform
Introduction
This document outlines the "Terms and Conditions of Collaboration for Physicians" on the MyMedEx platform. All physicians applying for registration must read, accept, and fully comply with these terms. Registration on the platform constitutes full and unconditional acceptance of the provisions below.
1. Legal and Professional Responsibilities of the Physician
1.1 All legal, criminal, and professional liabilities arising from any medical, diagnostic, consultative, or prescriptive services provided by the physician through the MyMedEx platform are entirely the responsibility of the physician. The platform shall bear no direct or indirect liability in this regard.
1.2 The physician agrees to provide services only within the scope of their licensed medical specialty and official credentials. In the event of any misconduct, negligence, medical malpractice, or violation of professional ethics, the physician shall be held solely accountable.
1.3 In case of patient complaints or disputes, all documents and communications may be submitted to the appropriate regulatory or legal authorities in the physician’s country of residence for legal proceedings or disciplinary review.
2. Service Commitments and Availability
2.1 The physician is obligated to attend all scheduled online consultation sessions (text, voice, or video) as defined in their personal dashboard, and to provide accurate, clear, and comprehensive care during those sessions.
2.2 In case of emergencies, delays, or inability to attend a scheduled consultation, the physician must inform the support team at least 6 hours in advance through the following official channels:

2.3 Failure to notify and absence from scheduled sessions without a valid reason shall be considered a breach of contract and may result in the temporary or permanent suspension of the physician’s profile.
3. Data Privacy and Confidentiality
3.1 The physician is responsible for maintaining full confidentiality of all patient data, medical records, and personal information, and agrees to use such information solely within ethical and legal boundaries.
3.2 Downloading, printing, sharing, or distributing patient data without explicit written consent from the patient is strictly prohibited, and may result in immediate account suspension.
4. Platform Intellectual Property
4.1 The physician acknowledges that the MyMedEx brand and all associated software, systems, content, technical infrastructure, and intellectual property belong exclusively to MedExFuture. Physicians shall have no ownership or usage rights over these assets beyond permitted operational access.
5. Termination of Collaboration
5.1 MyMedEx reserves the right to unilaterally terminate cooperation with any physician at any time in the event of repeated violations, inappropriate conduct, refusal to comply with platform guidelines, or breach of any of the terms stated in this agreement — without the obligation to provide justification.
6. Final Acceptance
By registering and beginning service delivery through the MyMedEx platform, the physician fully accepts all terms and conditions stated herein. The physician also agrees to comply with the laws and medical regulations of their country of residence, adhere to the highest standards of medical ethics, and follow all platform protocols.
